i have same problem, started recently, , it's pretty annoying. 

i did not have "download headers only" checkbox ticked, have , exchange account o365.

the issue occurs randomly on random attachments.

looks like, i'm not sure, happens more on low bandwidth connections.

yes, other mac users in organisation have same issue.

as mentioned, issue happens more on low bandwidth connections. identify it, please switch connection high bandwidth check issue?

meanwhile, please share following analysis.

1. detailed outlook version build. 

2. detailed steps duplicate issue.

3. screenshots symptom.

Outlook Rules not Running Automatically - Microsoft Community
